Choice Plan "A" (PPO)
• 90/10 (PPO); 70/30 (non-PPO)
• $5,000 stop-loss
• $20 doctor office co-pay (PPO); $40 doctor office visit co-pay (non-PPO)
• Prescription Drug Benefit:
  $10 or 20% of the cost (whichever is greater) for Generic drugs
$20 or 30% of the cost (whichever is greater) for Brand Name drugs on the formulary list.
$ 30 or 50% of the cost (whichever is greater) for Brand Name drugs not on the formulary list
• Wellness Benefit ($150 per insured/per year)
• New Chiropractic Benefit!
Choice Plan "B" (PPO)
• 80/20 (PPO); 60/40 (non-PPO)
• $5,000 stop-loss
• $20 doctor office co-pay (PPO); $40 doctor office visit co-pay (non-PPO)
